What to do to renew your ID:

 

Owner of Unit:

Contact your association (the company you pay assessments to) and request the

“Recreation ID Authorization Form”. Your association may require you to fill out a new census

card or other documents. Once you get the form, come to the ID location listed on the website (fourlakesmhoa.com/resident-ID). Please check the website first as the location of IDs can change based on the season.

 

Renter of Condo or Townhome:

Contact your landlord and ask him/her to request the “Recreation ID Authorization Form”. Their association may require a copy of anew lease, census card, etc. Once you get the form, come to the ID location listed on the website (fourlakesmhoa.com/resident-ID). Please check the website first as the location of IDs can change based on the season.

 

Renter of an Apartment (Towers or Abbey):

Go to your leasing center to get your letter. Once you get the form, come to the ID location listed on the website (fourlakesmhoa.com/resident-ID). Please check the website first as the location of IDs can change based on the season.

 

Please Note:

Associations may require additional paperwork to authorize your Recreation ID. Because of this it is recommended that you begin the process of getting your new ID one (1) month before your current ID expires.

UNFORTUNATELY, WE CANNOT ISSUE ID’S FROM UTILITY BILLS, MAIL, OR STATE IDS. WE

MUST HAVE THE LETTER IN HAND TO ISSUE YOU AN ID.
